| Heritage Place is flexible to meet the changing needs
of today's seniors. The Continuum of Care that is offered at our community,
from very independent to dependent, allows our residents to remain
at our community, which means, they deal with less change and stress. |

| As people age, generally, their needs grow. Here at
Heritage Place we are able to grow right along with those needs. The
opposite is also true. After a hospital or rehab stay, many seniors
are still in the process of regaining their strength and balance.
Residents at Heritage Place are able to receive a higher level of
care and once they are able to function on their own, we make the
switch to a more independent level of care. |
| Retirement Residences
: for active, independent seniors who
have had enough of the cooking, cleaning and yard maintenance |
| Assisted Living
: designed for seniors who are active and able, yet just need some
supportive services with activities of daily living. |
| Premium Assisted Living
: provides support for seniors who may need more assistance with the
activities of daily living. Assistance with walking and incontinence
available. |
| Memory Support :
Offers a secure environment to residents with Alzheimer's
and other memory problems that may tend to wander. Also additional
staff assist with the activities of daily living. |

Respite Services
|
| Short term stays, also known as "respite stays",
are available. When primary care givers of seniors will be traveling
out of town, or sometimes, simply need a break, we provide all the
services of our permanent residents to ease your mind. |
| We provide furnished, private apartments for respite
stays. Provided is: bed, night stand, dresser, swivel rocker, TV,
Telephone, linens paper products and soap. |
Heritage Place Assisted Living has made special arrangements
with the Davis County Council on Aging to provide funds especially
for respite stays.

Community Fee: $1250 one time fee
In order to maintain our common areas, provide the nursing evaluation,
prepare the apartments, and process paperwork, we require a one-time
community fee.
Meal Program: We include on emeal daily in the monthly service
fee. Residents may purchase additional daily meals on a monthly
basis. For each additional daily meal, simply add $150 per month.
Housekeeping Service: We include one housekeeping service
each week in the monthly service fee. Residents may purchase additional
housekeeping service for only $50 per cleaning.
